Registering with the Surgery

Our practice list is open and we are accepting new patients that live in our practice catchment area (see below). If you are a Temporary Resident please see below.

New Patients:

If you would like to register with our practice, please call in to reception during surgery hours.

Please bring your medical card with you. If you do not have your medical card, please fill in our permanent patient registration form, available from reception, or you may download a copy (See right).

You will need to make an appointment to see one of our Doctors for a registration check. Please bring a urine sample with you (bottles available from Reception).

Temporary Residents:

Our Doctors are happy to see patients who are temporarily resident in the Practice catchment area (see below) should medical advice be required. In this instance persons wishing to be treated should state that they are a temporary resident when reporting to reception.

To register you will need to complete our Temporary Resident Registration form which is available from reception, or can be downloaded from this website (See right).

Practice Catchment Area:

We register patients from the DE11 postcode area. For further information please contact us and ask the receptionist for details.
